risewellmt.com is a static informational website. It has no patient accounts, no medical intake forms, and no place to enter symptoms, diagnoses, medications, or other protected health information (PHI). Nothing you do on this site creates a medical record.
Appointment requests
The "Request an Appointment" buttons open your own email program addressed to risewellmt@outlook.com. Your message travels through your email provider and ours, not through this website. Please keep the request general (what you'd like help with and whether you prefer an in-clinic or telehealth visit); there is no need to include detailed medical history by email. Requests are reviewed by the Rise Well team before an appointment is confirmed.
Intake, consent, and your patient record
Once a booking is confirmed, intake questionnaires, consent forms, and your health record are handled through Athena (athenahealth), the clinic's electronic health record and patient portal, which is built for HIPAA-covered healthcare. Rise Well's Notice of Privacy Practices is provided to you as a patient through that process, not through this website.
Sunny, the site assistant
The chat assistant on this site answers general questions about the clinic (hours, services, the team, how booking works). It is not a clinician, does not give medical advice, and is not connected to any medical record. Messages you type are sent to a third-party language-model service to generate a reply and are not stored by Rise Well. Please don't share personal health details in the chat; save them for your visit.
Cookies, analytics, and hosting
- This site sets no advertising or tracking cookies and runs no analytics scripts.
- It is hosted on Netlify, which keeps standard server logs (such as IP address, browser type, and pages requested) for security and operations.
- Links to Instagram and to Athena take you to those services, which have their own privacy policies.
